Description
The overarching aim of the service will be to: Provide Care Act assessment and support planning services to a predefined number of service users as set out in chapter 6 of the Care Act Guidance. Provide Care Act reviews of support plans to a predefined number of service users as set out in chapter 13 of the Care Act Guidance. The provider will carry out person centred, strengths-based assessments and reviews, along with associated support planning, in accordance with Care Act 2014 principles and in line with Hampshire County Council policies which will be shared during mobilisation. This work is to be completed either in person with the individual being assessed/reviewed or on a remote basis (video call). The provider will not carry out a remote assessment if this would be inappropriate for the individual being assessed/reviewed. It is the responsibility of the provider to determine whether this is the case in conjunction with the individual and relevant people involved. Referrals will consist of community-based assessments for individuals living in their own homes or in registered care homes. Individuals referred will require Care Act Assessments or Reviews and may require Mental Capacity Assessments. All three pieces of work are to be treated as individual jobs. In total 150 assessments would be undertaken within the contract value.
