Bracknell Forest Council is planning the tender for a Home from Hospital service. We would like to draw on providers’ expertise to inform the review of the service model. This contract will cover residents of Bracknell Forest who require additional support to return home once discharged from hospital. The service will liaise with key stakeholders to receive and action referrals. A questionnaire is located, as an attachment, on the South East Business Portal https://sebp.due-north.com/ advert. Projected spend is between £70,000.00 and £80,000.00 per annum with a provisional contract duration of 4 years (2+1+1).
