Description
City and Hackney Integrated Commissioning Prevention Workstream are undertaking joint commissioning of two existing services; Social Prescribing (commissioned by the CCG) and 'Community Connections' a community navigation service (commissioned by City and Hackney Public Health). The new service will aim to provide a service that: - is an accessible and friendly service - provides personalised support - is for service users with non-medical needs who are at increased risk of future ill-health - enables service users to 'navigate' local services - enables service users to take control of their health and wellbeing and live independently The new service will build on the strengths of existing services and be fully integrated into other local provision as part of a Neighbourhood community navigation model including new Primary Care Network (PCN) social prescribing link workers funded by NHSE. Flexibility to respond to developments in community navigation will be a requirement of the service.
