Description
Dudley MBC is seeking provision of accommodation with housing related support for adult single people and couples aged over 18 years of age who are priority and non-priority homeless, at risk of becoming homeless or sleeping rough who need immediate access to housing and support in order to prevent homelessness in the Dudley borough The service must work positively and proactively with other accommodation-based services to identify a wide range of longer-term accommodation possibilities and facilitate access and referral to them. As part of individuals move on plans the service must include some transition work with appropriate community/floating support services where needed to ensure continuity of support to mitigate anxieties of moving onto independent living and setting up and sustain their new tenancy. All aspects of support within the service should enable individuals to live independently and sustain their tenancy or home and prevent the need for more intensive support. The service must provide person-centred, structured and outcome focussed support based on an individual's identified support needs and work positively and proactively with other appropriate services to ensure those support needs are met in a co-ordinated and holistic way. The goal being to prepare individuals for move on to an independent tenancy elsewhere.
