Description
CALL FOR APPLICATIONS FOR THE PROVISION OF A DEMENTIA SUPPORT SERVICE IN NORTH EAST LINCOLNSHIRE North East Lincolnshire Clinical Commissioning Group (NEL CCG) hereby invites applications from suitably qualified and experienced providers to provide a dementia support service in North East Lincolnshire for a 3 year period. The aim of the service will be to deliver a range of accessible, high quality, cost effective services that will promote, support and improve the mental, physical and emotional well-being of all people living with dementia within North East Lincolnshire. The service will provide advice, information and signposting to support people with dementia to participate and continue daily living activities important to them and to make informed choices on how they spend their time, encouraging them to maintain their independence, autonomy and functional abilities for as long as possible. Also, the service will promote, support and improve the mental, physical and emotional well-being of all carers of those living with dementia in North East Lincolnshire so they can continue in their caring role, look after their own health and well-being and have a life of their own in terms of opportunities for work, training, education, leisure and social interaction. Innovation and efficiencies in service provision for improved access and delivery of outcomes will be sought from prospective providers of the service, as will enhanced partnership working to provide an integrated service that works across health, social care, education, voluntary and other agencies within North East Lincolnshire, providing a seamless approach. The provider will be expected to deliver a service which is based at one central 'hub' in North East Lincolnshire with satellite 'spokes' throughout local communities, covering all GP federations to ensure those with dementia and their carers within the boundaries of NEL CCG are identified, recognised and supported.
