Description
In 2015, Liverpool Housing Trust (LHT) made an application to the Homes and Communities Agency (HCA) through the Affordable Housing Programme 2015-18 Prospectus, to obtain funding for a supported accommodation housing scheme, located on Field Lane, Fazakerley. The Council has supported the scheme and proposes to nominate current Knowsley clients including those who currently live out of borough receiving social care services to be the beneficiaries of the scheme, in order to maximise positive outcomes for clients through cost-effective means. The proposals will provide 10 one-bedroom apartments, 5 two-bedroom wheelchair accessible bungalows and 1 one-bedroom bungalow, which will accommodate a maximum of 21 clients with Learning Disability/Autism, Physical and/or Sensory Disabilities. The Service Provider of the supported living arrangements will support adults with one or more of the following disabilities: • Acquired brain injury • Physical or sensory disability • Autistic spectrum disorder • Learning disability including complex, cognitive and physical needs • Enduring mental health difficulties • Young people in transition with a disability and/ or learning disability Each category may include people with complex co-existing needs e.g. difficulty engaging with services and leading a chaotic lifestyle, including drug and alcohol misuse, mental health problems, a learning disability and/or possible forensic history. Support will be available 24 hours a day, 7 days per week. The provider will also be required to provide on-site management as part of the service and senior management available on 365 days per year.
