Community Led Homecare - Expressions of Interest

Cheshire West and Chester council are looking to speak to local community organisations that could support with community led homecare. Working closely with adult social care and hospital discharge teams the provider will provide a holistic offer to vulnerable and older people in the community The provider must have capacity and capability to deliver services across the week and weekends including bank holidays The provider will adopt an enablement approach to support people to remain independent and able to live in their homes.
