Provision of a Targeted Support Service for local people following a Stroke and /or who are living with Dementia in North Lincolnshire

North Lincolnshire Council is seeking information on the delivery/provision of targeted support service for adults who have had a stroke and/or have dementia. The ambition is for the targeted service to complement existing services for Adults and their carers as part of the North Lincolnshire Single Organisational Model.
