CTP 771 For the Delivery of a self-management programme for living with long term conditions

The Authority is inviting tenders to provide a self-development programme for people living with long term conditions (?Programme?), to empower people that require support to manage their long term condition through a self-management model. The Programme must be available to people within the geographical area of the County of Derbyshire, with the exception of Derby City (?County?).
