Description
City & Hackney continues to demonstrate an on-going commitment to place-based integration. The C&H Place Based Partnership has made great progress in bringing services together so they are organised around eight Neighbourhoods. City and Hackney adopted more of an asset based approach that is focusing on what matters to residents; working more closely with local communities; and taking a more proactive approach to identifying and supporting residents who have complexity in their lives. The City & Hackney Neighbourhoods programme funds local health, social care and voluntary sector providers to deliver towards the above aims and to reorganise their services around a place-based model. The City & Hackney Place Based Partnership funded a Community Pharmacy provider to deliver the following non-clinical support to the Community Pharmacies in local the Neighbourhood: • Support the roll-out and neighbourhood engagement of GP referrals to the Community Pharmacist Consultation Service. This includes providing training for GP support staff, engaging with residents and community groups, promoting the use of the new Health First minor ailments scheme and monitoring number of GP CPCS referrals by neighbourhood. • Work collaboratively with neighbourhood teams to deliver a successful COVID-19 and Influenza vaccination programme, focussing on the vaccination of the residents in care homes and residential homes. • Work within the neighbourhoods to deliver greater population health through preventive care work with a focus on measuring/ monitoring blood pressure, healthy lifestyle weight management, exercise, alcohol & smoking issues, and promoting self-care advice and self-help. • Work with each primary care network to generate a specific neighbourhood action plan where community pharmacy can support system partners. • Ensure the workforce needs of Community Pharmacy are considered as part of the development of the Neighbourhoods organisational development plan. • Support the evolving pathways of the Neighbourhoods programme including anticipatory care, Multidisciplinary Meeting - including Mental Health Multidisciplinary Meeting and blended teams and Children and Young People pathways.
