Description
1.1 The service will provide support for adults with mental health issues to progress their mental health recovery and to live in a way that is based on hope, confidence and the ability to live a rich and full life. 1.2 The service will support clients to meet the outcomes - agreed in their Support Plans. The support should assist people to build up skills, abilities, confidence, support networks and community activities that enable them to achieve greater independence/interdependence. 1.3 It is intended that support will be delivered in such a way that the need for longer term Council funded support will be reduced or removed. 1.4 It is intended that the service will achieve the following outcomes: • Clients are satisfied that the support they receive enables them to recover a life that has hope, meaning and purpose. • Clients have access to support, that is appropriate, delivered in a timely manner and responds to fluctuating needs • Clients meet the Recovery goals set out in the Support Plans that they have co-created • Clients are able to live interdependently within the community with appropriate support (accommodation, employment, social integration) • Clients are able to reduce their need/use of health and social care eservices 1.5 This provision is for people with unmet eligible social care needs and therefore in receipt of a personal budget, and who have asked the Council to arrange their services. 1.6 This is a service for adults who are ordinarily resident in Sheffield. (with exceptions for adolescents in transition from Children's, and for whom Sheffield is the responsible commissioning local authority for their social care provision).
