Nutrition and Dietetics Service in West Hertfordshire

NHS Herts Valleys Clinical Commissioning Group is seeking providers interested in delivering a Nutirition and Dietetics service in West Hertfordshire. The Community Nutrition and Dietetics Service provides care to patients with dietetic related conditions in the community through the creation of a management plan, monitoring of symptoms and teaching self-management in order to reduce need for additional care and maximise quality of life. Qualified and accredited dietitians will operate the service within community clinics, patients' homes, care homes etc. The service may also operate in an acute setting, providing specialist dietetic input in a multi-disciplinary team if necessary.
