Description
Wakefield Council is reshaping the provision of services to improve health and wellbeing, by procuring a social wellbeing service. The Service will have two key priorities; to increase health and wellbeing and promote self-management. The Service will deliver the following direct measurable outcomes to improve health and wellbeing in the local population as a whole: Self-reported health and wellbeing is improved People are equipped with the confidence and tools to have a good life Knowledge and uptake of services to increase wellbeing, self-management and independence is increased People feel less isolated and have more opportunities for social interaction and participation People stay independent for longer due to increased access to information and better coordinated services The tender documents are available for interested suppliers to download from the YORtender system (www.yortender.co.uk) using reference A7QE-62JB3N.
