Lincolnshire Health Check Audit and Support Services

The current service model offers software plus additional support for the delivery of the NHS Health Check programme across Lincolnshire. This is further outlined below; 1) The software element provides the GP practices with a list of eligible patients to invite, keeps track of the number of invitations sent, records the data required at each NHS Health Check and the number of completed assessments. The anonymised patient data is reported back to the commissioner. The software provides activity data, validates this data for payment and provides a myriad of outcomes reports. 2) The support element provides an annual audit of each practice. The practice data is audited to check that all of the data required is being collected at every NHS Health Check and is compliant with the national offer. This support offers staff training at the time of the audit and as required in-house when new staff start or require a refresher to keep compliancy high. The service also supports the NHS Health Check programme by writing the GP systems templates, working with the software provider to improve the ease of use for practices and resolving any other software issues.
