NHS Wandsworth Social Prescribing Service

The Social Prescribing Service will: - improve social and clinical outcomes for people with long-term conditions and their carers; - make more cost-effective use of NHS and social care resources; - enable Clinicians to refer patients to a range of local, non-clinical services to improve their health and wellbeing; - address people's needs in a holistic way and facilitate access to the right place, first time; - contribute to building a more supportive, inclusive and resilient community.
