Description
NHS Calderdale and Kirklees Clinical Commissioning Groups (CCG's) are seeking to commission a Community Ophthalmology Services through an Any Qualified Provider (AQP) process. This process will qualify providers to deliver services to the eligible practice population of Calderdale and Kirklees. Services available to providers in this tender are listed below: 1. Pre and Post-Operative Cataract Service 2. Glaucoma Referral Refinement Scheme 3. Minor Eye Conditions (MECs) previously Primary Eye Acute Assessment Referral Scheme (PEARS) 4. Ocular Hypertension (OHT) Monitoring The services below can only be delivered to the eligible practice population of the North Kirklees Primary Care Network footprint: 5. Low Vision Aids Community Service 6. Specialist Contact Lens Service
