Description
Lancashire County Council is looking for an organisation/s to provide a Hospital Aftercare Servcie. This service will offer low level support for those people who are able to be discharged from hospital, but it is felt that they would benefit from some assistance on returning home. The main objectives of the service are to: • Support hospital discharges • Reduce pressure on Social Services • Reduce instances of re-admission to hospital The Service will operate across two Tiers: Tier 1 - Take Home and Settle Tier 2 - Time limited service for up to 3 hours direct support that includes transport home and offers low level support with the intention of helping an individual settle back into their own home.
