This service is an integrated rehabilitation pilot for people with co-occurring mental health and substance use needs, including: - Joint mental health and substance assessments, development of holistic care plans, and delivery of integrated interventions for individuals with co-occurring severe mental illness by secondary mental health psychiatrists and social workers. - Coordination of mental health and substance use care, including clinical care, by designated mental health care coordinator. - Provision of personalised budgets for individuals with co-occurring mental health and substance use needs to address immediate basic needs and support further social and emotional development.
