Provision of Independent Support - Pre-Qualification Questionnaire

Manage or maintain an Independent Support service in any given local authority area where trained independent supporters are available to be deployed from 1 July 2016 - 31 March 2017. The role of Independent Supporters will be to provide families and young people up to age 25 with timely and appropriate support through the EHC assessment and planning process.
