The LCH Independent Investigation Team have accessed a wide range of evidence from multiple stakeholders including NHS organisations, regulatory bodies, and individuals (staff, patients, patient's relatives/representatives). Information provided includes documents, written and oral evidence including, where relevant, personal data, in order that the Investigation Team can robustly deliver the Terms of Reference.
