Mental Health Social Inclusion Service

The purpose of the service is for the service provider will seek to assist adults (18 years and over) with an identified (functional) mental health problem within a recovery model. The service provider will be closely linked into local mental health services and will work in collaboration to ensure a coordinated approach to managing the client's mental health. Supporting positive outcomes around employment, education, housing and wider social inclusion.
