Home Support Immediate Response

Re-ablement services currently form part of the councils in house provider services offer. The service is offered to service users who require a period of support to regain independent living skills following a period of ill health, injury or hospital admission. The service forms a key part of the hospital discharge process ensuring those people medically fit for discharge can be supported at home and avoiding delayed discharges.
