Description
Bracknell Forest Council's Adult Social Care, Health & Housing is looking to procure an outcome-based, flexible and sustainable Community Support Service: (previously known as Domiciliary Care or Home Care service) - Supports people to regain or attain independence to an optimum level. -Maximises and maintains people to live independently for as long as possible. - Provides value for money and addresses the containment of costs. This drive to maximising independence for the people supported will require a significant cultural change both within the council, the individual and the provider market. In particular we are looking for Providers to partner with the voluntary and community sector creatively and flexibly to meet the needs of people. It is anticipated that through this approach, the level of care provision will reduce over the life of the contract. In so doing the department is seeking to commission a community based support service for adults who reside in Bracknell Forest through the procurement of a framework contract with 5-9 organisations.
