Description
Hampshire County Council is undertaking a pilot to inform the design of the future NHS Health Check programme across Hampshire from April 2019. Participating practices will be opted out of the current health checks system. Primary Care Practice Groups/Alliances will need to have a population of around 17,000 40-74 year olds to achieve a monthly invite rate of around 200 eligible people (approximate list size of 40,000). Financial and technical support will be provided in the period up to the pilot start date and will continue throughout the pilot duration. A tiered payment structure, based on evidence of the outputs from the health check, will be implemented during the pilot phase. The pilot will be carefully evaluated to aid development of the future Hampshire NHS Health Check programme due to commence from April 2019. This is a real opportunity for local Primary Care providers to help shape the future delivery of NHS Health Checks in Hampshire and improve outcomes for practice populations.
