EoL/Palliative Care Transport - NHS Walsall CCG

The aim of the service is to transfer individuals safely within the Walsall area between home and care homes either for respite or for their healthcare needs to be met. For end of life individuals it could be their preferred choice of end of life care. In exceptional circumstances only and with agreement of the Commissioner, the Provider can be booked for an out of area hospital appointment if all other avenues of transport have been explored; including the individual's mobility entitlement or car and these are not available or appropriate.
