Call-off from Framework Agreement for the Provision of Outreach NHS Health Checks LOT 2-Outreach Service

Award to a provider (already on the Framework Agreement) to provide outreach NHS Health Checks which aim to engage key target groups who are at higher risk of cardiovascular disease and who are less likely to be reached by NHS Health Checks provided by GP practices. The target groups will vary over the period of the contract according to need. The contract was awarded for 2 years with the option to extend for a further 2 years.
