Community Living Support Networks Service - AWARD

Community Living Support Networks are a successful model of social inclusion for vulnerable single adults in need of support to maintain or sustain independent living. The Contractor will deliver Community Living Support Networks based on the model outlined in the service specification, addressing any barriers to independent living and build community cohesion. The model is based on a vision of included communities, where individuals with additional needs are empowered to thrive. The Contractor will build and strengthen networks of family, social and locality-based support that will prevent housing and health crisis and will respond proactively to long-term needs to prevent costly escalation and dependence. Submitted Tender Response Documents will be evaluated in accordance with the award criteria and methodology outlined in the ITT document. The ITT Document will be made available to suppliers who Express Interest on the Sandwell Metropolitan Borough Council Tendering Portal.
