NHS Frimley ICB ("the Commissioner") sought to commission provision to provide a Social Prescribing Service The service provides assessment of needs, motivational support, goal-setting and multiple referrals to other suitable services for those people where the referring clinician believes that social, emotional and financial issues are directly causing or contributing to deteriorating health conditions.
