Description

This procurement is being conducted in accordance with the Health Care Services (Provider Selection Regime) Regulations 2023. Hampshire County Council has a requirement to meet its statutory public health duties by taking steps to improve the health of the population. This is supported by the Hampshire Public Health Strategy ambition to help residents to pursue Healthy Lives and in particular support residents maintain a healthy weight and be physically active. Hampshire County Council's Public Health is seeking to procure a tier 2 adult weight management service designed to meet the needs of eligible Hampshire residents, based on National Institute of Health and Care Excellence (NICE) guidelines. The service will be in two lots. Lot 1: Universal service a) A tier two weight management service, delivered face to face, at accessible venues across Hampshire along with a remote service (group or individual service) for people unable to access physical venues. The Provider will be expected to provide the service at a minimum of 70 locations across the County of Hampshire. b) A physical activity based tier 2 weight management service in which the behaviour change components of the tier 2 model are delivered alongside a physical activity intervention (co-determined by Commissioner and Provider) for male-only, female-only and mixed-sex groups. The Provider will be expected to provide the service at at a minimum of 70 locations across the County of Hampshire. Lot 2: Specialist service Specialist tier two weight management services for defined health inequalities groups a) A specialist tier two weight management service, for people living with a severe mental illness. The service will be delivered face to face, at accessible venues across Hampshire. The Provider will be expected to provide the service at a minimum of five locations across the County of Hampshire. b) A specialist tier two weight management service, for people living with a learning disability. The service will be delivered face to face, at accessible venues across Hampshire. The Provider will be expected to provide the service at at a minimum of five locations across the County of Hampshire Lot 1 Service Description: The Provider will develop and deliver a clear pathway and service model which provides a flexible and responsive outcome-focused service compliant with NICE guidance. The provider will market the service, establish and promote referral routes into service and triage referrals. The Service must be compliant with current and future NICE guidance. The Provider will deliver an evidence-based intervention of at least 12 consecutive weeks duration and must use a variety of behaviour-change methods, be multi-component and include structured goal setting and monitoring of behavioural and weight loss goals. Support should also be provided to participants via a digital platform. Lot 2 Service Description: The Provider will develop and deliver a clear pathway and service model which provides a flexible and responsive outcome-focused service compliant with NICE guidance. The provider will market the service, establish and promote referral routes into service, including outreach in relevant communities and triage referrals. The Service must be compliant with current and future NICE guidance. The Provider will deliver an evidence-based intervention of at least 12 consecutive weeks duration and must use a variety of behaviour-change methods, be multi-component and include structured monitoring of behavioural and weight loss goals through the use of individualised plans. The programme must be delivered in small groups of up to 15 participants. The Service must have appropriate adaptations for individuals with mental health conditions, learning disabilities, or special educational needs and disabilities.
