Description
The Community and Lifestyle Weight Management Service will ensure the delivery of outcome focussed, high quality and person centred obesity prevention and weight management services for adults (including pregnant women) and young people aged over 14 (with parental consent), across Wakefield and district. The Service will commence on 1 April 2018 and will be put in place for three years to 31 March 2021. The Council may extend the agreement for a further period of one year, making the final possible expiry date 31 March 2022. The estimated total value stated includes additional funding of £25K per year (£100K over the life of the agreement) to allow for fluctuations in demand of Service Users accessing the Service. The Council is unable to guarantee that this additional funding will be used, demand will be based on the needs of the Service Users accessing the Services.
