Approved Provider List (APL) for Supported Living for Learning Disabilities across 5 Lots. Lot Description 1 Lot 1 – Community Support • A few hours of support per week to develop/maintain independence. • Focused on connecting and enabling and most aspirational in terms of enablement. • Similar to homecare, delivered as an outreach model to help people live independently in their own homes. Providers encouraged to develop this service from existing hubs/schemes if appropriate; or as an extension of their homecare business. 2 Standard Supported Living with variable day hours • Where background support is not required at all times; night support not usually required. Significant potential for support requirements to flex up and down; requires flexible shared hours that are not a barrier to people moving in. • Service users likely to require ongoing support but also needs to be a focus on connecting and enabling people to maximise independence. 3 Standard Supported Living with variable day hours and night support • Where background support is required most of the time during the day and at night. Support requirements may reduce and some flexibility is required around shared hours but unlikely to change significantly for the majority of residents in this service. • Still enablement focused but support likely to be more consistent 4 Enhanced/Intensive Supported Living with 24/7 support • For people who need complete support all the time to achieve outcomes. Support requirements unlikely to reduce significantly and more intensive, 1:1 support required with most aspects of daily living. • Focused on supporting in a way that minimises increases in need and enables people wherever possible. 5 Specialist/Intensive Supported Living • For people with very complex care/support needs – e.g. ‘Transforming Care’, where support is likely to be 2:1 or higher and a high degree of staff skill, training and management oversight are required to continue to meet outcomes and ensure placements remain stable. Call-offs likely to be for individuals.
