Primary Care Support to Care Homes

NHS Bexley Clinical Commissioning Group (the CCG) is seeking expressions of interest from providers and the market in general. We are keen to hear from potential providers who are able to deliver primary care support in a care home setting which can be either residential, providing personal care needs, or nursing homes which have trained nursing staff to deal with more complex clinical needs. This service will be delivered by a general practice multi-disciplinary team under the oversight of a GP to patients who are registered with a GP practice in the borough of Bexley.
