NHS South Kent Coast CCG Mental Health Local Care

This procurement is for a mental health within local service to operate within the four CCGs in East Kent (NHS Ashford CCG , NHS Canterbury & Coastal CCG, NHS South Kent Coast CCG and NHS Thanet CCG). It is envisaged that the service will work within the new models of care supporting people's mental health and working in partnership with other Local Care services providing a Multi-Disciplinary Team (MDT) approach for their mental health along with other needs, including physical health, employment, housing and social care and would have a greater emphasis on prevention, self-management, choice, peer support, and partnership with other sectors. The service will provide assessments, short term interventions and care navigation from GP practices and Local Care Hubs.
