Description
Mole Valley District Council (MVDC) is looking for a supplier to deliver the East Surrey Outreach Service (eSOS) throughout the geographical areas of Mole Valley District Council, Reigate & Banstead Borough, Epsom & Ewell Borough Council and Tandridge District Council. The purpose of the service is to enable rough sleepers, sofa surfers and homeless vulnerable and socially excluded people including those with mental health problems and complex needs to: o access accommodation with or without support; o prevent and/or relieve homelessness; and o prevent, reduce and delay health and care needs and costs. The service will also offer: o assertive outreach for vulnerable people sofa surfing, rough sleeping, or at risk of rough sleeping; o support to people placed in assessment beds provided by Leatherhead Start (http://leatherheadstart.org/); o interventions to prevent former rough sleepers and sofa surfers who have secured accommodation from being evicted; o resettlement support to former rough sleepers and sofa surfers who have secured accommodation;
