Description
The Council wants to work with a support provider to deliver a Younger Adults Supported Accommodation in Havant for working age adults who have a Learning Disability, Physical Disability/Sensory Impairment, Mental Health Condition and/or Autistic Spectrum Conditions. Access to the services will be via a Care Act 2014 assessment. It is important for the Council to build relationships with a support provider that genuinely wants to work in partnership to actively explore, present and deliver service innovations that better meet the needs of clients, the service and the Council. The support provided will focus on developing greater self-reliance and independence and below is an indication of the range care and support that may be required by individuals living in the Younger Adults Supported Accommodation. Younger Adults Supported Accommodation (Havant 4) is made up of an initial 2 x 2 bedroom properties that will be used to accommodate 2 individuals with complex needs who benefit from having their own self contained accommodation. There may be future expansion of the Service if there is a requirement to support other individuals in the same area who also need similar individualised accommodation. The properties consist of an open plan living room/kitchen and dining area, 2 bedrooms upstairs and a bathroom upstairs. There is a larger bedroom which will be for the tenant and a smaller bedroom for the sleep in/staff accommodation. The properties have front and rear entrances and each have their own gardens. There are parking spaces allocated to each property. The properties are in the Havant area and are close to local amenities including local bus routes and a large area of park land. The Service requires support from an organisation which has extensive experience in supporting individuals with more complex needs including people with a range of learning disabilities.
