LPS CARE - NHS Health Checks Community Outreach Service - LCC24596

"NHS Health Checks are currently provided in Lancashire through 135 general practices and a third-party provider who delivers health checks across the county in community pharmacies, community settings, workplaces, and places of worship. Lancashire County Council Public Health are seeking to procure a new service which includes an integrated hybrid delivery model. This will promote working in a more collaborative way between the third-party provider and general practices. Please see the tender documents for more information in relation to the Maximum Tender Price per year, incentive payments and price uplift. The Authority proposes to enter into one Contract with the successful Tenderer Initial period of five years with a right to extend the contract for up to 36-months beyond the initial term by any number of defined periods as long as the total contract term does not exceed eight years.
