Multiple Needs Navigator Service - Bridge Project

To ensure people with multiple and complex needs have access to appropriate health and wellbeing care, including alcohol and drug treatment and recovery, and mental and physical health services. The service also offers support to access settled accommodation, form positive and healthy relationships and access to education and employment.
