Description
Kingston Borough Council is seeking expressions of interest from suitably experienced and competent service providers for the delivery of the Community Outreach NHS Health Check Programme. The overall aim of this service is to improve the health outcomes and the quality of life of the eligible population by providing health checks in easily accessible community settings. This will support the delivery of the programme in Primary Care by providing a flexible health check service that can be accessed opportunistically. The focus will be to engage the hard to reach groups, including those that do not engage well with general practice, to enable them to pro-actively reduce their risk of cardiovascular morbidity, premature death or disability and improve the quality of their lives. The anticipated annual contract value is £40,000 per annum and is expected to start on 8th August 2016. The duration of the contract will be eight months.
