Wrap Around Care Service

The aim of the wrap around care service is to support people to return, and remain, at home following a stay in hospital by delivering 24/7 care in the person’s own home for an intensive short period (between 2 and 10 days). This should prevent, or delay, the admission into residential care and promote personalisation and independence. The service will support individuals to regain and maintain activities of daily living, and thereby reduce reliance on long term care and support and help them to return to live at home, as independently as possible.
