Description

Solihull Council currently commissions a Dementia Support Service to provide support for people in Solihull who have dementia and their carers. The service includes: o A Dementia Navigator Service: The role of the Dementia Navigator Service is to provide support across the whole span of an individual's dementia journey, working directly with carers even when the person with dementia does not take up support or is unable to participate. The support is tailored to each individual according to their needs, this may include: o Advising people where they can access support following a diagnosis. o Providing access to activities which are suitable. o Assisting people to self-manage their condition. The intensity of support is provided on a tiered basis as follows: o Tier 1 - telephone and email support. o Tier 2 - more intensive face-to-face support. o Tier 3 - staying in contact with people, with access to increased support available if required. And o A post-diagnostic activity group The post diagnostic activity group is a weekly peer support group for 15 to 20 people with dementia to participate in activities with people in similar circumstances, to share experiences and aid people to come to terms with their dementia diagnosis and maintain their independence for longer. The Council is carrying out a review of its Dementia Support Service as the current contract is due to end on the 31st March 2026. The Council wants to engage with the market to support the shaping the future dementia support service before progressing to retender this service.
