Description
NHS Greater Huddersfield Clinical Commissioning Group are wanting to appoint a provider to provide safe, high quality, effective GP led primary medical care and health promotion. Tailored to the unique needs of specific populations living in the Greater Huddersfield area who are registered with the practice. The provider will provide support to patients to enable a large propoirtionto transfer to a mainstream provider of primary medical services within a 12 month period. The population covered will include: Homeless Asylum seekers Refugees Sex workers The average number of patients registered with the practice should be 1500. The registeredlist should not exceed 1750 or drop below 1250 patients, unless with prior agreement. The list size must be managed proactively to ensure that: The practice can service its population adequetly It moved patients into mainstream practice throughout the year rather than en-masse It maximises the use of the resources
