Description
Wakefield Council has developed a new model of provision for Housing Related Support. Some of the elements of this model are being procured as part of this contract. The contract has been split into the following 3 lots: Lot 1: Assessment Pathway The Assessment Pathway will comprise of: • Preventing Rough Sleeping Service • Assessment Centre • Short term Supported Housing Lot 2: Access to Housing Pathway - Main Provision The Access to Housing Pathway will provide housing related support to people who are at risk of homelessness & need support to both find & maintain suitable housing. The Pathway will support people to find & access accommodation & then provide outcome-focussed housing support in order to enable them to gain the skills to live independently. Lot 3: Housing Sustainment Pathway The Housing Sustainment Pathway will provide housing related support to people who are at risk of homelessness to enable them to keep & maintain their housing. Bidders may bid for one, some or all of the lots available, however the Council reserves the right to only award one lot to a bidder. The contract will be for an initial 4 year term, but at the Council's discretion the contract may be extended by up to a further 3 year period.
