Covid Triage Service - Single point of access (SPOA) and Navigation Networks

Community navigation networks and single point of access, are central to enabling the integrated model of community navigation in City and Hackney. Community navigation is an umbrella term we use to describe a range of roles that provide 1-2-1 non-medical person-centred support to residents, to improve wellbeing through addressing wider social determinants of health. It is much more than simply signposting, and involves empowering residents to make the most of their personal and community assets, and ensuring they are accessing all the support they need in a coordinated way.
