Staywell Frailty Service

The purpose of this procurement is to fill the gap in service provision of support services for people who live alone and do not require social care but would benefit from interventions to promote their independence, keep them safe and well at home and connected with their local communities. There is a need to provide increased support to targeted people who are assessed as frail, and who are therefore at risk of deterioration and increased need for health and social care services.
