Description

The Dementia Post Diagnosis Support Service will act as a key support to enable people to live well throughout the disease journey. The aim of the Dementia Post Diagnostic Support Service is to be the main point of contact for the person diagnosed and their carer during the entire dementia journey. Potential bidders will be expected to demonstrate an awareness of and the ability to provide support at all stages (including complex, challenging and end of life/bereavement support) of the disease. The service will provide appropriate emotional support, practical facilitation, guidance and information to meet the needs of those entering the service, as well as facilitating access to well-being interventions which promote independence and quality of life. The service will commence on 1st April 2018. The contract will be for three years duration. Commissioners reserve the right to extend for up to a further 24 months. The value stated represents the value should the contract be extended for the full 5 year term. The Services to which this ITT relates fall within Chapter 3, Section 7, Particular Procurement Regimes to the Public Contracts Regulations 2015 and Part H of the Annex V to the Public Contracts Directive (also known as "light touch regime" formerly Part B services). Neither the publication of the above advertisements, the inclusion of a Bidder selection stage nor the use of the term "Pre-Qualification Questionnaire" nor any other indication (including any statement in this ITT) shall be taken to mean that the Contracting Authority intends to hold itself bound by any of the Regulations, save those applicable to Chapter 3, Section 7, Particular Procurement Regimes.
