Description

The London Borough of Hounslow is a diverse and growing community in west London, with residents speaking over 188 different languages. Our population is increasing and living longer, which, combined with rising levels of long-term conditions and dementia, means demand for adult social care is both growing and becoming more complex. The provision of home care for residents of 18 years of age and over, with noncomplex conditions and who are ordinarily resident in the London Borough of Hounslow (LBH). The home care service is being jointly commissioned by the Authority and West and North London ICB. The core purpose of the home care service is to enable people to live more independent and healthier lives by giving them greater choice and control over their care and support options, maximising their social support systems and strengthening support in the community.
