Description
West Suffolk CCG is currently reviewing eye services within the west Suffolk locality. The overall vision is for an integrated eye health service across our CCG. To date, a number of draft enablers and draft pathways have been developed to support the vision of an integrated service. This is your opportunity to give feedback on the draft pathways and the opportunity on how we could procure these pathways. This feedback will inform the final integrated ophthalmology model for west Suffolk. A number of key enablers have been identified for this service; •Consultant-led triage, •Shared patient records that are held on an IT platform with the capability to integrate with other systems •A strong focus on education and training to up-skill optometrists in the community. The enablers and pathways have been identified to facilitate a framework that allows for further innovation and integration. From triage, a number of key pathways will cover a range of conditions; •Glaucoma •Cataracts with direct listing for surgery and post-operative follow-up, •Children’s pathway and •Minor eye conditions. For the glaucoma pathway, West Suffolk CCG is seeking to develop a consultant-led service with repeat testing, starting treatment and reviewing stable/low risk patients in the community. This is an innovative change in the current pathway. You are invited to an exciting ophthalmology review and comment session on Tuesday 1 March 2016 at 5 pm. This event will offer the opportunity for you to meet the commissioners and provide feedback on the proposed model and pathways. The event will be open to all stakeholders, with an interest in ophthalmology including primary, community, secondary and tertiary clinicians and providers, wellbeing services, IT software providers, orthoptists, Optometrists, the voluntary sector, members of the public and patients. You are encouraged to offer their views on the overall future model, the enablers, the draft pathways and the opportunities to procure the integrated service. The event will also facilitate time for you to renew or make new connections with an aim to encourage partnerships that could help to deliver an integrated and the responsive network within West Suffolk, which will enable the vision for services for the coming years.
