Discharge to Access (D2A Beds)

The City of York Council of the City of York and the NHS Humber & North Yorkshire Integrated Care Board (the "Commissioners") are looking to commission up to 10 Discharge to Assess (D2A) Beds on a block purchase basis across a range of care home facilities to support our patients and customers following discharge from hospital on discharge Pathway 2.
