Description
The Council is seeking to engage a provider who will deliver a combined and cohesive service for all children, young people, and their families (0-19) who are resident or attending school in the Durham local authority area should receive the Healthy Child Programme. The service will be available for young people with a special educational need (SEND) and/or disability and those who are Care Leavers up to the age of 25. Transitional support to young people into further education and low-level mental health and emotional wellbeing support for young people aged 19-25 will be available to young people through community support.
