Hampshire County Council invites capable providers to tender for the provision of Public Health Nursing 0-5 years (Health Visiting and Family Nurse Partnership) The Health Visiting service leads delivery of the Healthy Child Programme 0-5 years (HCP 0-5). The HCP 0-5 is an evidence based prevention and early intervention public health programme offered to all families resident in Hampshire and lies at the heart of the universal service for children and families. It aims to support parents, promote child development, reduce inequalities and contribute to improved child health outcomes and health and wellbeing. The Family Nurse Partnership is a licensed, evidence based, intensive nurse-led prevention and early intervention programme for vulnerable first time young parents and their children.
